Check 21 Disclosure

Based on the proposed rule, effective October 28, 2004, this Check 21 Act Disclosure is required to be given to consumers who receive checks back with their statements, request a copy of their check to provide proof of payment, or have a check returned for insufficient funds. This disclosure is available in English or Spanish and can be printed on these designs.
