SIMPLE Employer Document—Designated Financial Institution

Form 5305-SIMPLE is an IRS model SIMPLE plan document. An employer may use this plan document in combination with the SIMPLE IRAs to establish a SIMPLE plan. It should only be used by an employer with a financial organization that agrees to serve as the DFI.
