SIMPLE Notification to Eligible Employees

This form can be used by an employer to notify its employees prior to the 60-day election period that they can make or change a salary reduction election, and to inform them of the employer's intended contribution for the forthcoming plan year. This form also notifies an eligible employee that a SIMPLE IRA must be established to accept contributions and custodian/trustee information provided to the employer.
