    SEP Documents

    SEP Plan Agreement (SEP-ERC)
    An employer can use the SEP-ERC to establish a SEP plan for a business. The SEP-ERC includes Form 5305-SEP, Instructions for the Employer, and Information for the Employee. Eligible employees must establish traditional IRAs using Forms 5305 or 5305-A, available as Organizers.

    SIMPLE IRA Employer Documents (SIMPLE-5304, SIMPLE-5305)
    An employer can use Form SIMPLE-5304 or Form SIMPLE-5305 to establish a Savings Incentive Match Plan for Employees of Small Employers (SIMPLE) IRA plan for the business. Together with the Model Notifications to Eligible Employees, Model Salary Reduction Agreements, Article VI Procedures for Withdrawal, and Election to Transfer Without Cost or Penalty forms, a business owner will have the documents necessary to establish and operate a plan. Eligible employees must establish SIMPLE IRAs using either Form 5305-S or Form 5305-SA, available as Organizers.

    SIMPLE IRA Organizers (IRA-SIMPLE-C, IRA-SIMPLE-T)
    This form helps you save time and money by combining several forms into one easy-to-use document. The SIMPLE IRA Organizer is available for custodial accounts and trust accounts. It helps you collect necessary information about your SIMPLE IRA owner and supplies the IRS model Form 5305-S or Form 5305-SA to officially establish the SIMPLE IRA. The Organizer includes an application, required disclosure statements, and financial projections, and helps document your SIMPLE IRA owners’ beneficiary designations and accept the initial contribution.
