    Account Opening Documents

    Traditional IRA Organizers (IRA-49-BSL, IRA49T-BSL, IRA-49-SD, IRA-49-CU)
    IRA Organizers help you save time and money by combining several forms into one easy-to-use document. The Traditional IRA Organizer is available for traditional custodial, trust, and self-directed IRAs, and in a credit union-specific version.

    The Traditional IRA Organizer helps collect necessary information about IRA owners, and supplies the IRS model Form 5305 or Form 5305-A to officially establish an IRA. The Traditional IRA Organizer includes an application, required disclosure statements, and financial projections, helps you document beneficiary designations, and accept the initial contribution. IRA owners can add or change beneficiaries after account establishment using IRA Designation or Change of Beneficiary—Including SIMPLE IRAs (IRA-1).

    Traditional IRA Booklets (IRA-BKLT, IRA-BKLT-T, IRA-BKLT)
    The Traditional IRA Booklet supplies the IRS model Form 5305 or Form 5305-A to officially establish a traditional IRA and includes required disclosure statements and financial projections. The Traditional IRA Booklet is available for traditional custodial, trust, and self-directed IRAs. Traditional IRA Booklets are designed for use with the Traditional IRA Application (IRA-49-A).

    Contribution, Rollover, and Transfer Documents 

    IRA Contribution Instructions for Traditional and SIMPLE IRAs (IRA-24) 
    These instructions provide a convenient way to obtain necessary contribution information, such as owner information, IRA identification, type of IRA contribution, and tax year of the contribution.

    Traditional IRA Contribution Receipt (IRA-25)
    This form may be used to document a traditional IRA contribution.

    IRA Rollover Review for Traditional and SIMPLE IRAs (IRA-16) 
    This review helps an IRA owner certify that assets are eligible for rollover. Through a series of simple yes and no questions, an IRA owner can avoid some of the common mistakes that may invalidate an IRA or eligible retirement plan rollover.

    Request for Direct Rollover to a Traditional IRA (IRA-DIR-ROLL) 
    This request authorizes an eligible retirement plan administrator (employer) to directly roll plan assets to an established traditional IRA.

    Request for Transfer to an IRA for Traditional and SIMPLE IRAs (IRA-TRANTRAD) 
    This request authorizes another custodian/trustee to transfer assets to an established IRA of the same type.

    Distribution Documents

    IRA Distribution Form for Traditional and SIMPLE IRAs (IRA-4/W-4P) 
    Make documenting distributions simple. This form records the amount withdrawn and the reason for the withdrawal, as well as establishes scheduled payments for normal and pre-age 591/2 distributions. It includes IRS Form W-4P for the withholding election.

    Traditional IRA Distribution Receipt—IRA-17 
    This form may be used to document an IRA distribution. It does not include IRS Form W-4P.

    IRA-50 and IRA-50/W-4P ) 
    These forms help an IRA custodian or trustee satisfy the requirement to provide a notice of the right to change or revoke any previous IRA withholding election.

    Age 70½ Documents 

    IRA Age 70½ Calculation Worksheet for Traditional and SIMPLE IRAs (IRA-46)
    This work sheet helps an IRA custodian/trustee complete and explain an IRA owner’s required minimum distribution calculation.

    IRA Age 70½ Election Form for Traditional and SIMPLE IRAs (IRA-6)
    This comprehensive document allows an IRA owner to establish his/her election of payment for required minimum distributions upon attaining age 70½. It includes IRS Form W-4P for the withholding election.

    Waiver of IRA Minimum Distribution (IRA-44A)
    This form documents an IRA owner’s intention to take the required minimum distribution from another IRA.

    Report of Required Minimum Distribution and Report of Required Minimum Distribution Amount (IRA-RMDRPTA and IRA-RMDRPTNA)
    These forms will help an IRA custodian/trustee meet the requirement to notify an IRA owner of his/her required minimum distribution status and/or amount for the year.

    Amendments

    Traditional IRA Amendment (Standard-Custodial, Trust, Self-Directed), (Conversion-Custodial, Trust, Self-Directed)
    Standard Traditional IRA amendments include the applicable IRS series 5305 form and are used to update IRA owner documentation when there are significant changes to IRA rules and regulations and/or as directed by the IRS. Includes a built-in cover letter explaining the changes to the IRA owner.

    Conversion Traditional IRA amendments are designed for use when an IRA custodian/trustee changes IRA document providers. Sending a conversion amendment to existing IRA owners will ensure that all accounts are being administered under the same provisions.
